DOCUMENTING WINDOWS OF HOPE CONTRIBUTIONS

To satisfy IRS requirements, the Windows of Hope Family Relief Fund will need certain information to properly issue receipts to businesses or individuals who donate cash or property for the October 11th event.

Berdon LLP - our accountants and advisors - will coordinate the financial documentation. The checklist below will assist you in gathering the necessary information. Use the attached forms provided by Berdon to complete the documentation.

PROMOTIONAL MATERIALS

For any businesses or individuals who ran promotions for the October 11th event, please supply the following:
1.   Copies of any solicitation or promotional materials you used to publicize the event.
2.   Copies of any advertisements you ran.

CONTRIBUTIONS OF CASH

For any cash contributions received from individuals, please supply:
1.   The amount of cash contributed.
2.   The name and address of the donor.

Use the Cash Contribution Log to compile information

CONTRIBUTIONS OF PROPERTY

For any noncash contributions from individuals, please supply:
1.   A description of the property.
2.   The value of the property.
3.   The name and address of the donor.

Use the Noncash Contribution Log to compile information

AUCTIONS, ETC.

For any fundraising event - such as a silent auction - involving the sale of goods or services for the benefit of the Relief Fund, please supply:
1.   A description of the property or services sold.
2.   The value of the property or services sold.
3.   The selling price
4.   The name and address of the purchaser.
